This was the lead unit of the IHJNS-28 that ran a restricting signal and rear-ended another intermodal train (ILBMNB-29) on the night of October 1, 2011. As the two trains collided, this locomotive rode up onto a well car, struck several containers and derailed the last 5-unit well. Judging from the paint on the nose, at least one of those containers was green. The remains of some of the containers and well car can be seen at right. The other two units involved in the accident were UP 4285 and UP 2198. In a slight twist of irony, the unit appears to have recently had PTC equipment applied to top of the cab. One injury reported.
