Don't blame the engineer- I see a black void, the rust stain coming from it and the discoloration below that indicates the defect had been growing for a while. Likely a bad casting. Neat photo.

I agree, I can see the bad casting too. Besides, the knuckle is the designed weak point, so if the knuckle breaks you can blame the engineer, but if something besides the knuckle breaks don't you dare blame it on him.

First of all when you're going at a grade and the yellow is on level ground you stop at the yellow YOU NEVER STOP ON A GRADE or this will happen
